Job Description
PERFORMANCE PLANNING 
Responsible for overall operation of the DCH Diabetes and Nutrition Education Center. Oversees the planning, implementation and evaluation of the diabetes self-management education program of out-patients as well as in-patients. Assures the department’s alignment in achieving the organizational expectations/goals related to the organizational priorities and strategic objectives. Provides direct patient care and education. Supervises professional and support staff. 
KEY RESPONSIBILITIES / ESSENTIAL FUNCTIONS 
  1. Plans, evaluates, and revises curricula, course content, course materials, and methods of instruction as per evidence-based standards
  2. Achieves continuous systematic, measurable improvement in areas of responsibility.
  3. Sets performance standards for staff. Develops working environment and avenues for education and training to meet those standards.
  4. Develops marketing strategies for increasing volumes as well as physically visiting the physician offices regularly (annually or more often).
  5. Assists with program support functions as needed. Represents DCH and the DCH Diabetes and Nutrition Education Center to the community and promotes public awareness of diabetes education at programs and health fairs.
  6. Provides education and training in diabetes self-care management to patient and family(as available)
  7. Carries out individual assessment of patient health and diabetes status, identifying learning needs
  8. Develops individualized education and training plan for diabetes self –care management.
  9. Identifies patients having special problems in learning or in diabetes management and who require additional training
  10. Provides instruction in group setting for those who are appropriate candidates.
  11. Documents assessment, education plan and patient progress
  12. Generates written report on patient status and teaching to the referring physician or agency
  13. Documents patient visit for statistical and billing purposes.
  14. Participates as a professional team member of the DCH Diabetes and Nutrition Education Center
  15. Participates in performance improvement activities.
  16. Identifies the educational needs of others, assists in developing formal educational or training plan or classes, and teaching or instructing others.
  17. Provides leadership to maintain program recognition by the American Diabetes Association
  18. Supervises, evaluates and strengthens the in-patient Diabetes Education program
  19. Keeps abreast of developments in the field by reading current literature, talking with colleagues, joining professional organizations and participating in professional conferences.
  20. Manages departmental budget
  21. Interviews, selects, hires, and retains employees
  22. Ensures orientation and training for employees
  23. Manages performance
  24. Promotes, demotes, or transfers employees to meet organizational needs
  25. Executes progressive discipline up to and including termination
  26. Manages departmental productivity standards
  27. Approves payroll and is responsible for accurate payment of employees
  28. Manages front line operations of area of accountability related to meeting/exceeding departmental goals, 5keys of excellence, DCH Mission and Vision. 
DCH Standards: 
  • Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
  • Performs compliance requirements as outlined in the Employee Handbook
  • Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
  • Requires use of electronic mail, time and attendance software, learning management software and intranet.

MINIMUM KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED 
Current Alabama RN Licensure.  Bachelor’s degree in Nursing required. Academic preparation and/or experiential preparation on program management. Academic preparation and/or experiential preparation in the care of persons with a chronic disease. At least two years of nursing experience. Certified Diabetes Educator (CDE) OR, must meet the diabetes education contact-hour requirements of the American Diabetes Association Education Recognition Program AND, acquire teaching experience to meet eligibility requirements to take CDE exam. Take and pass CDE exam within 4 years of hire date/transfer date. Current American Heart Association Healthcare Provider BLS Certification. Must be able to read, write legibly, speak, and comprehend English. Good communication and teaching skills. Legible handwriting.  Basic word processing and computer skills.  Able to use standard office equipment such as telephone, computer, copier, and fax.   
WORKING CONDITIONS 
WORK CONTEXT
  • Must be able to engage a client interaction for up to two hours or a class session for up to three hours.
  • Must be able to educate and document on a patient within the two hour block of time. 
  • Must be able to complete all documentation after teaching a large group of patients.  
  • Must be able to respond to and teach a wide range of client ages, abilities, backgrounds, and needs. 
  • Must be able to tolerate clients with poor hygiene or wounds. 
  • Must be able to respond to clients who are sick during a session and need medical or family assistance.
  • Includes possible exposure to human body fluids, disease, infection, lab chemicals and hazardous materials and/or cleaning solutions
  • Requires wearing of common protective and safety equipment such as gloves, safety glasses, mask, gowns, etc.
PHYSICAL FACTORS
  • Is able to stand, walk, kneel, sit, crouch, or stoop with good balance for extended periods of time.
  • Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work
  • Must be able to perform the duties with or without reasonable accommodation.
  • Hearing and vision must be normal or corrected to within normal range.
  • Physical presence onsite is essential.
